Excessive cost. Forced to overpay due to circumstances. Exorbitant fees. As having money pulled from your nose would be painful, so t his phrase implies a merciless payment for a necessity. The idiom usually refers to circumstances that one has no other options. (Compare to   Between a Rock and a hard place.)

“When my car ran out of gas in the desert, I had to pay through the nose for a tow truck to bring me more gas.”
